IMPORTANT: INSTRUCTIONS FOR COMPLETING THE FORMS
If using the Word version:
- Download the Word form and open with MIcrosoft Word
- Type the required information into the form where indicated
- Type your name in the Signature Box or add your signature as a picture
- Save the completed document as a .docx or .PDF format
- Download the PDF form and open the document with Adobe Reader
- Use the 'Fill & Sign' tool to complete the form where indicated
- Type your name in the Signature Box or add your signature as a picture
- Save the completed document (PDF format)
- Download the preferred format form and print it
- Complete in black ink
- Scan the printed page 1 or photograph it with a mobile phone
- Ensure the resulting image is legible
